Your Community Hospital case manager will make help you transition back into your home by arranging for any services or medical equipment that you may need. If you require continued care or rehabilitation, the case manager will make those arrangements, too.

According to the Case Management Society of America, the primary goal of the Case Management Department is to be a patient advocate, empowering patients and families by providing them with information to make informed healthcare choices.

Community Hospital's case managers are actively involved with discharge planning to ensure patients receive continuity of care and are discharged to a safe environment. Anticipation of post-hospital needs and appropriate discharge planning are priorities for the case managers.

Discharge planning meetings on a regular basis. Information is gathered from all departments involved with the patient, and discharge plans are formulated. The primary physician is actively involved with the discharge planning process.

Conditions of participation require all Medicare facilities perform utilization reviews of all patients admitted to the hospital. The case manager reviews the patient’s medical record to ensure patients meet criteria for admission. Reviews also include an evaluation of appropriate treatment and use of hospital resources. The Colorado Medical Foundation provides guidelines and criteria for hospital admission and discharge screens.

For private insurance patients, the case manager is responsible for communicating with the insurance company to obtain authorization for the hospital stay and assisting with any discharge needs.

Donna previously worked at Lea Regional Medical Center, providing all aspects of case management including UR, social services, D/C planning, referrals and education. She is a certified professional in utilization review (CPUR). She received her nursing degree from New Mexico Jr. College in Hobbs, N.M.

Lynda is certified in case management. Prior to joining Community Hospital she worked in the insurance industry doing case management. She attended nursing school in Arizona and practiced in Arizona and northern California. She has worked in a variety of nursing positions, including intensive care units and outpatient clinics.
