Community Hospital began offering cardiology services at Grand Valley Cardiology in 2020. The following year, Community Hospital opened the Cardiovascular Procedure Center, a state-ofthe-art Cardiac Cath Lab to provide high quality health care to patients with heart conditions, including cardiac rehabilitation. 

What Should You Expect?

Orientation: Staff will review your medical history and lifestyle habits and help create appropriate goals. 

Exercise Testing and Prescription: One of two exercise tests will be performed to establish a baseline to thoroughly track your progress in the program and create an appropriate exercise plan. 

Exercise: An individualized workout plan will be created from the exercise testing. We’ll teach you self-monitoring concepts for use in the Cardiac Rehab gym as well as outside activities. 

Education: Bi-weekly educational classes are offered to anyone attending our Cardiac Rehab program. Various wellness topics will be discussed by our integrated group. 

Support Network: Patients have the opportunity to work with others in the cardiac rehab program with similar diseases and share experiences. This unique camaraderie provides an additional layer of support throughout the program. 

Candidates for Cardiac Rehabilitation

If you have experienced any of the following, you are a candidate for cardiac rehabilitation: 

  • Heart Attack (Myocardial Infarction) 
  • Stable Angina 
  • Coronary Artery Bypass Graft Surgery (CABG) 
  • Percutaneous Transluminal Coronary Angioplasty (PTCA) 
  • Coronary Stenting 
  • Valve Repair/Replacement 
  • Chronic Stable Heart Failure 
  • Ventricular Assist Device 
  • Cardiac Transplant
